We are looking to support an innovative, accessible service whereby five hundred people complete a validated questionnaire to assess their risk of diabetes. Those at high risk should have point of care testing HbA1c to determine diabetic / non diabetic hyperglycemic status. This contract will be in place from 1st October 2015 – 31st March 2016 Additional information: All clarification questions must be clearly marked CLARIFICATION with the question and Provider details clearly set out. Any clarification questions from the Provider to the Council should be sent to (please note if you do not register your interest then we will not be able to send you the up to date clarification question and answers).
