The prime contractor will be responsible for the provision of the whole cardiac pathway and will also be responsible for sub-contracting elements of the services to its partner providers. The Prime Contractor shall be directly responsible for managing the total pathway including the clinical outcomes, key performance indicators and financial payments to its sub-contractors. It is not envisaged that one provider alone will provide all the services required, as this would impact on the rights of patient Choice, and also the whole-system need for service management. It is envisaged that the “One Stop” model of community-based cardiology will be redesigned. Newham’s Joint Strategic Needs Assessment (JSNA) and commissioning intentions identify cardiac care as a high priority for redesign, building on the achievements of the current community cardiology service. The Newham vision is for care to be more integrated and accessible, to promote earlier diagnosis and better care; to empower patients to self-manage; and to reduce the incidence of CVD and its associated complications. The key elements of the new service will be: A requirement for the Integrated Community Cardiology Service (ICCS) to be staffed by a Multidisciplinary Team consisting of consultant support and oversight , GPwSI, specialist cardiology nurses and Heart Failure Nurses. It should be noted that Newham’s proposed service requires fundamental changes in pathways, including how patients are treated. It is not an outreach clinic from secondary care or another intermediate service. The key differentiators include: • Referral management. • Skill mix. • Community location. • Closer working between primary and secondary care. • Engagement of clinicians. • Telephone advice to primary care. Core activities will include, but not limited to: • Clinical Assessment of patient • Set up and implementation of Referral Triage service • Full diagnostic service and reporting of results within scope of service • Patient advice and education • Follow up at a subsequent clinic appointment if necessary • Clinic audit for each patient seen Additional information: NHS Newham CCG is keen to test the potential provider market for Cardiovascular Disease Services. Potential providers with relevant technical capability and experience to deliver this service are required to express an interest if they are interested in bidding for this service should the CCG decide to carry out a competitive tender exercise and publishes a Contract Notice for this requirement.
