The range of services to be included within two totally independent contracts (Lot 1 - Derbyshire County Council and Lot 2 - Derby City Council) are to enable easy access to appropriate information and advice for individuals with Dementia and their carers, to develop a range of community groups and to facilitate peer support networks. We are seeking providers with a proven track record of managing and delivering high quality services alongside health and social care partner agencies, to clients eligible to receive these particular services.
