This is to provide Outcome 5 of the Public Health target and support the Health and Wellbeing Strategy. The Family Focus Health Trainer provider(s) will: - Establish a Health Trainer Working Group - Implementation of the Health Trainer pathway - Identification and implementation of the referral criteria and protocol, including exclusions (A family will need to consent to being referred into the programme otherwise this will be considered an exclusion.
