Initial 6-month pilot, followed by benefits assessment, and potential longer-term on-going service. Caseload up to 60 normal patients, plus 9 non-weight-bearing. Covers discharge from three acute sites: KCH (Canterbury), QEQM (Margate), WHH (Ashford) . Budget for pilot is £1.1M. D2A ‘Home First’ - model provides an opportunity for patients to be transferred form an acute hospital bed in a timely way to an appropriate alternative community setting for on-going assessment and rehabilitation. The model is focused on a more proactive management of frail older people and improving their long term outcomes.
