For those patients with a rapidly deteriorating condition that may be terminal the Fast Track process was set out in the National Framework to enable immediate provision of NHS Continuing Care and it is the PCU’s intention to ensure that we continue to commission care that improves patient safety and quality. The Lead Provider would be required to coordinate the fast-track requirements of the CCG and sub-contract work to geographically appropriate care providers, thereby offering the commissioner a single point of contact and taking overall ownership of the delivery model. The engagement events will be designed to explain this approach in detail and it is hoped potential providers will help to develop the model before we formally approach the market. Space at both events are unfortunately limited so each provider will be restricted to a maximum of 3 attendees.
