The service will support all patients into recovery (in accordance with their individual recovery goals) whether they have long term enduring mental health conditions or are nearing a point of crisis. Initial appointments will involve an assessment of the person's needs using the Outcome Star tool and/or Warwick & Edinburgh scale, as well as a comprehensive risk assessment. Pro-active promotion and engagement and will ensure that the service is reaching the community where the need is greatest.
