Communities of practice are groups of practitioners who come together to provide peer support, shared expertise and experience in developing a common purpose. In this instance, the common purpose is designing and implementing new models of acute care collaboration. There are 13 acute care collaboration vanguards and of these 13, seven are working on accountable clinical networks, two on specialty franchises and four on hospital groups models.
