The Commissioner is seeking a Lead Provider able to deliver a Service which can meet the health needs of the local populations of Shropshire CCG and Telford and Wrekin CCG through a Community based service delivery model and use of innovative service redesign. ### The Service The Commissioner wishes to receive responses to the Invitation to Tender (ITT) from suitably qualified and experienced Ophthalmology providers with the necessary capacity and capability (or a demonstrable ability to provide the necessary capacity and capability) to provide a Community Ophthalmology Service. The Service shall deliver a 'one stop shop' service, with diagnostic and clinical assessments being undertaken at the same appointment, to ensure timely access to specialist clinical expertise in a community setting. The Service Provider shall be expected to work with the Commissioner to develop the processes necessary for further ophthalmic conditions to be treated/managed in a Community Based Ophthalmology care setting throughout the duration of the contract. ### The Service shall:- • Provide timely access to specialist clinical expertise in a community setting. • Deliver a 'one-stop shop' service with diagnostic and clinical assessments being undertaken at the same appointment. • Improve access and reduce waiting times for ophthalmic services, with a flexible appointment system to meet the needs of Service Users and carers. • Provide advice, information and support to primary care practitioners. • Support and enable Service Users to manage their own conditions by providing a supportive and proactive approach to self-management and to signpost Service Users to services that can support them to self-manage. • Ensure Certification of Vision impairment is completed and provision of support aids where required. • Provide a Service that is multi-disciplinary and patient focused. • Provide access to high quality, safe clinical care that gives timely advice, appropriate support, assessment, diagnostics and treatment for patients according to their individual need. • Provide care closer to home, where appropriate. • Commission a high quality service that integrates with other providers. • Generate a reduction in unnecessary out-patient appointments. • Create a Service that uses innovative methods of service delivery.
