The evaluation has 4 aims: • To evaluate the effectiveness of current GP LES coverage and provision • To define and determine the benefits of the current provision to the care homes resident population • To undertake an options appraisal to develop an optimal model for future provision, and cost the model • To scope capacity in city GP practices for delivery of an optimal model across all city care homes. The objectives of the evaluation are to: • Determine the effectiveness and benefits of current provision under the GP LES. • Recommend a feasible, costed optimal model for Nottingham City Care Homes. • Review capacity within the current structure to deliver 100% coverage of Nottingham City Care Homes against an optimal model. The value of the contract is £19k including VAT.
