We are governed by member GP practices and lead the development of the local healthcare system with our partners and the people of Camden. Over the last few months we have been working with the Design Council on a key target; reducing the number of unnecessary clinical episodes by proactively diagnosing hypertension before a critical event takes place such as a stroke or heart attack. ### PROJECT OVERVIEW There are over 18,000 patients at risk of hypertension in Camden whose condition could be better managed if they were identified before an acute healthcare episode occurred. The challenge is to develop a holistic social marketing campaign targeted at patients and staff of the 36 Camden GP surgeries to proactively measure the blood pressure of individuals at risk of hypertension. GP surgeries have devices (called surgery pods) enabling patients to independently measure BP and are financially remunerated for doing so.
