The Hampshire Clinical Commissioning Groups will achieve this through partnership arrangements with service providers that embrace an outcomes based, personalised approach to support. This is a developmental specification. Service providers will accept and acknowledge that aspects of this specification may change. The Service will be co-produced, personalised and outcomes focused, providing support predominantly to service users with Learning Disabilities and Autism Spectrum Conditions, and also to service users with Physical Disabilities, Sensory Impairments, Mental Health Problems, a Forensic History and other Vulnerable Adults (16 years or older) to include (but not limited to) the following service models: • Support to individuals which may include the provision of personal care • Shared support to two or more individuals which may include the provision of personal care • Overnight support to individuals or groups which may include the provision of personal care • Community Support Networks • Support to access employment, education or other vocational activities • Landlord Supported Housing Additional information: 1.
