The Health Assessment will include the following: -Height / Weight / BMI -Waist and Hip Measurements -Blood Pressure (BP) -Assessment of dietary intake -Assessment of Physical Activity (PA) -Assessment of Alcohol consumption -Smoking status -The One you assessment and Heart Age Calculator and Lung age monitor tools will be used as engagement and illustration tools to clients. -Dementia Awareness All Health Assessments and Health Check bookings should be coordinated with targeted workplaces/businesses and the current NHS Health Check provider. The Contractor will also be required as appropriate to promote other Public Health campaigns such as flu vaccination.
