Our population is increasing and living longer, which, combined with rising levels of long-term conditions and dementia, means demand for adult social care is both growing and becoming more complex. The provision of home care for residents of 18 years of age and over, with noncomplex conditions and who are ordinarily resident in the London Borough of Hounslow (LBH). The home care service is being jointly commissioned by the Authority and West and North London ICB.
