The service is for vulnerable Wiltshire residents (aged 18+) who are medically fit for discharge but unable to return home due to practical barriers. The service runs from Monday to Friday with flexibility to support over weekends when there are Hospital pressures. The service is responsive and works effectively with a range of partners to facilitate discharges. The successful service provider will have skills in navigating and coordinating solutions that prevent an individual being discharged home.
