We want to use this linked data effectively to produce robust analysis on the patterns, risks and drivers of poor health and health inequalities within our population. We require a provider that can deliver both actionable insight that drives improvement work and tackles inequalities in health, and also upskill the capabilities of our PHI Analysts to embed Population Health Management (PHM) approaches in our work.
