The aims of the LTC community outreach and EPP programme are to deliver cost-effective, culturally competent, and preventative community health interventions that address the health inequalities in Black, Asian and Minority Ethnic (BAME) communities which constitute circa 50% of SWL ICB's core20 communities, particularly in deprived and underserved wards such as Thornton Heath and New Addington. In particular the LTC community out-reach health and wellbeing checks, Pentathlon healthy lifestyle courses and Expert Patient Programme (EPP) LTC self-management course are anticipated to: - Support raising grass roots awareness of LTC risk and risk factors and support people to adopt healthier lifestyles to reduce the risks of LTC, support early identification and diagnosis and self-management in key Croydon communities. - Empower patients with LTCs to feel more confident in managing their own condition and keep them active and engaged in their lives. - Tackle health inequalities and increase uptake from hard-to-reach groups.
