Where this cohort of patients no longer require an acute hospital bed and have been identified as unsafe to return home immediately (even with services), these Complex Dementia D2A beds will enable citizens to receive ongoing care and/or rehabilitation for up to six weeks. During this time, health and social care assessments can take place in an appropriate setting, at the right time for the patient. Additionally, these beds will also support step-up from the community where admission into an acute setting can be avoided.
