The current contracts for Reablement Service (Intermediate Care - Pathway 1) in Somerset expire at the end of August 2025 and we have a commissioning plan underway to have a new contract(s) in place from 01st September 2025. Over the last year we have been working with Newton Europe to design new approaches and tools to support flow and effectiveness, as well develop a range of reporting that enables providers to both case manage and deliver good outcomes.
