This service is jointly funded by BHCC Public Health and NHS Sussex to support the integration of health and care in a way that supports person-centred care, sustainability and better outcomes for people aged 50 years and over across the city. The AWS will be open to anyone aged 50 years and over but will target those people who are identified as being most at risk of a decline in their independence and wellbeing.
