The service will be based around delivery in general practice plus some community delivery outside of general practice with resources targeted to people who are likely to benefit the most. The aim of the service is to improve the health and wellbeing of adults aged 40-74 through the assessment and management of the major risk factors for cardiovascular disease. The provider will proactively seek to address inequalities in access to and outcomes from the NHS Health Check by working with other local partner organisations. The service will prioritise supporting people to understand and manage their risk rather than focus on case finding people with previously undetected conditions.
