The GP-led specialist service undertakes holistic Frailty Assessments for a proactively identified patient cohort and develops person-centred care and support plans, including advanced care planning where appropriate, for those individuals. The service is delivered in the patient's own home or in a clinic setting, based on individual's need. The service provides both 'routine' and 'rapid access' appointments.
