The key objectives of the Procurement are to establish providers who can host Social Prescribing Link Workers to provide a proactive, preventative approach to social prescribing that will identify patients who would benefit most from the support. Social prescribing link worker (s) will work as part of a Primary Care Network or part of the Locality Mental Health Team and will collaborate with colleagues as part of a wider multi-disciplinary team.
