The provider will also be required to deliver core Primary Care and associated prevention, protection and wellbeing services to meet the needs of the local resident population. The Commissioner is keen to explore the opportunity for APMS/GMS providers to combine core primary care approaches with wider community partners offers in support of improved health outcomes and associated wider benefits to the local community. Community approaches developed with communities and a focus on the wider determinants of health and improved user experience is sought.
