This will be in line with assessed levels of need and may form part of a continuum of care. It is intended that the service shall: ● Build capacity within the home environment and support families to develop strategies and skills to assist with the management of the child’s needs and/or condition(s); ● Enable children and young people to access social and leisure opportunities, assisting them to integrate into local universal and specialist community resources; ● Enable family carers to take a break from their caring role. The family carers may choose to remain in the home or to take a break outside of the home.
