The service will take referrals for clients who are engaged with Adult Social Care, all clients will have been assessed under the Care Act and will have a suitable care or support package in place to meet any identified needs. Staff at the project will monitor the wellbeing of clients and ensure that access for care staff and adult social care staff is facilitated.
